Courtesy of Oak Street Health Oak Street Health, a chain of primary care centers for adults on Medicare, just opened its second Long Island location. The company opened a primary care clinic at 210 Fulton Ave. in Hempstead. The Hempstead location follows the 10,000-square-foot clinic at 14-16 Brooklyn Ave. in Freeport that opened in July. The new Hempstead center is the 18th Oak Street Health location in New York State. The company says it is the only primary care provider to carry the AARP name.
