Courtesy of Oak Street Health Oak Street Health, a chain of primary care centers for adults on Medicare, will soon open its second Long Island location. The company plans to open a primary care clinic at 210 Fulton Ave. in Hempstead on Friday, Oct. 13. The Hempstead location follows the 10,000-square-foot clinic at 14-16 Brooklyn Ave. in Freeport that opened in July. The new Hempstead center will be the 18th Oak Street Health location in New York State. The company says it is the only primary care provider to carry the AARP name.
